During a transition into the home from the hospital or assisted living rehabilitation setting, Associate Home Health is committed to bringing professional health care services directly to you. We can provide service to patients with an illness or disability, recovering from surgery, or those who maintain a household status that compromises their health. Our staff of nurses, therapists, and aides provides excellent home health care to residents of the greater Chicagoland area including the counties of Cook, DuPage, and Lake. We offer an alternative to nursing homes and institutions by catering to our patients' needs in the comfort of their home. Below is an overview of the services we provide in a nurturing home environment:

Skilled Nursing Services

Initial and periodic nursing assessment of patient, medical care of homebound patients with skilled medical needs, diabetic instruction and management, medication management, follow-up surgery care, Hi Tech infusion care. We also provide specialty wound care treatment such as Wound Vacuum therapy for optimal improvement.

Home Health Aide Services

We can provide assistance with hygiene, grooming, and bathing.

Physical Therapy

Therapeutic modalities for relief of joint and muscle pain, exercise to help restore muscle function and range of motion to obtain maximum potential in attaining increased functional mobilities, family and caregiver education with home exercise regimen and safety training. We can also provide in-home Ultrasound therapy for rehabilitation.

Occupational Therapy

We can provide instruction in self-care (dressing and bathing), in homemaking, or in learning to live more fully with disability, assessment of home environment with regards to patient safety.

Speech Therapy

We can provide help with speech and hearing problems such as those caused by a stroke.

Medical Social Worker

We can provide assistance to clients/families with sorting out personal, family, work, financial and other problems that may occur as a result of illness or disability.

Other Services

      The physician certifies the medical need for skilled care and establishes a plan of treatment to be provided at the patient's home. Every sixty two days the service is evaluated and a determination is made to alter or maintain the home care program.
